Registration Checklist

Please complete and/or submit each item below.  

1. BC Education I.D. Number.
2. Copy of Birth Certificate.
3. Recent report card and/or .
4. Proof of Residency - (e.g. Utility bill / Driver's License).
5. Registration Form - All sections of the online registration form completed and confirmed.
6. Grade 10-12 First Assignment completed.
7. Completed course selection sheet (please download PDF file using link below and fax or mail to FVDES).

Preferred payment methods are Interac, credit card, money orders or certified cheques. Please note that uncertified cheques will no longer be accepted due to the high incidence of NSF cheques.
