FVDES School Computer Acceptable Use Form
Student & Parent Contract / Computer Access Agreement
This form concerns student use of school computers.
We the student(s) enrolled in the Fraser Valley Distance Education program and the parent(s) or legal guardian(s) of that student, understand that the FVDES Online! Program (hereinafter referred to within this document as "the Program") at the Fraser Valley DE School (hereinafter referred to within this document as "the School"), is committed to providing to each student the best possible education, utilizing computer technology and the Internet / World Wide Web. In consideration of the School providing to the student an email account, a computer (on loan for the duration of student enrolment) and various software, all of which are the property of the School, we understand and agree that it is in the interests of the student, the School and the Program that the following policies be followed and adhered to by us in regards to this technology:
- NO person shall at any time, access, alter or delete any computer system files or application files.
- NO person shall re-engineer, re-configure or re-code the computer hardware or software, in any way, except with the express permission of the Technical Support staff.
- NO person shall contaminate either the hardware or software on the computer by way of introducing a computer virus to the computer or, damage or destroy the hardware or software or any accessories provided.
- NO person shall install any programs, including any chat programs, on the School's computer, unless so directed by either an FVDES instructor or FVDES Technical Support personnel.
- ALL persons shall respect the computer hardware and software, and all accessories, textbooks, etc. assigned to the student for the term of enrolment in the Program, understanding that it is all the property of the School.
- ALL persons shall use the Internet, the World Wide Web and other Internet resources for educational purposes only.
- ALL persons shall inform the Technology Staff immediately if, at any time, the computer equipment fails to work or does not work properly.
- ONLY the STUDENT and/or KEY PARENT may use the equipment.
- The Registrar (jgoldsmith@k12connect.com) of the FVDES Program must be informed immediately of any changes of address &/or phone number(s) for student, parent and computer. I.e. any relocation of the equipment must be made known to the school.
- The Technical Support Dept. of FVDES must be informed immediately if the FVDES site passwords become known to people OUTSIDE of the school,
We understand and agree that failure to comply with these policies or any violation of them by us may be, at the discretion of the School, cause for immediate expulsion of the student from the School and result in the immediate seizure of all equipment provided to us by the School. We also understand and agree that, in extreme cases, criminal charges shall be laid where the law is broken.
We agree to make any further restitution necessary for any additional charges resulting from misusage or mistreatment of the equipment, as outlined within this document.
Refund Policy (Early Withdrawals / Prior to June 30th of School Year)
Students who withdraw from FVDES 'early' will receive a refund on the condition that the computer & textbooks are returned to the FVDES offices in a timely manner. Parents / students are expected to return ALL materials within one week of withdrawal.
Students using FVDES computers who withdraw 'early' from FVDES will be charged a computer cleaning fee of $25.00.
Students who are asked by Administration to return computer equipment as a result of their failure to complete assignments and/or tests (lack of Progress) will forfeit their computer refund UNLESS the computer is returned within one week.
We have read, understand, and agree to abide by all of the above policies and the terms under this Agreement. We agree to accept the educational, financial and legal consequences of the failure of the student or the failure of the parent or guardian or the failure of both the student and the parent/guardian, to follow any of the policies or terms contained in this Agreement.
